Job Description

Summary of The Role

This role involves leading and supporting a care team to deliver high-quality, person-centered support for residents. Key responsibilities include providing guidance and motivation to staff, overseeing care planning and compliance through systems like Nourish, coordinating medical and external stakeholder involvement, and ensuring all documentation is up to date and reflective of individual needs. The role also includes facilitating resident engagement, supporting training and competency, managing audits, and handling practical duties such as food orders and healthcare tracking. Collaboration, leadership, and attention to detail are essential to ensure consistent, compassionate, and compliant care delivery.

Role Responsibilities

Ensure positive team working by providing motivation, support, guidance, and advice to the team members.
Work alongside the PBS Team to develop site activities/events.
Support PBS Specialists and Lead to make referrals to LD teams and IST teams.
Ensure all Support Plans/Risk Assessments and all documentation on Nourish (care plan systems) are reviewed each month and are maintained to be accurate, person centered and written in 1^st
Work alongside the individual and key stakeholders to ensure all care plans and mental capacity assessments and best interests are completed.
Complete daily Nourish compliance checks for daily records and the weekly audit.
Work collaboratively with the shift leaders to ensure all appointments/medical concerns are responded to appropriately
Lead on all MDT meetings taking minutes of meetings and upload to Nourish Care Plan
Complete person-centered resident meetings weekly and record and support actions on the tracker and review the actions with relevant employee, i.e. Shift Leader/PBS Co-ordinator.
Ensure everyone we support has person centered outcomes and review these each month on the nourish system
Interviewing
Staff competency training – annual fire, buccal, knowledge and hand hygiene booklets.
Complete food orders for each home reflective of individual requests from resident meetings
Update the weight tracker
Update the healthcare matrix
Update all PEEPS and Carry out fire grab bag audits monthly

Requirements

Essential Skills, Knowledge & Experience

NVQ level 3 in Health and Social care (desirable)
Professional manner
Knowledge and experience of working with vulnerable adults with learning disabilities.
Calm natured, patient, and optimistic.
Good written and communication skills
Sound Knowledge on Microsoft/excel systems.
Knowledge of ACC and Nourish (Desirable but training will be provided)
Good problem-solving skills
Flexible working methods
Ongoing involvement to develop professional development

Maintain confidentiality of information relating to people being supported, staff and the company.
To work in line of company policies and procedures and government legislation.
To maintain and manage records in line with company policies and procedures, Data Protection and GDPR guidance.
Attend regular Leadership Meetings and Staff Member Meetings, take minutes if requested by the Service Manager.
Attend regular supervisions and yearly appraisals with the Service Manager.
Work closely with the team and those being supported to gain a better view and perspective on the support being provided.
Show consistent dedication and contribution to the development of the service.
Be flexible and responsive at all times to meet the needs of the service, the people being supported and the team.
Be available to work flexibly to meet the needs of the service; this includes evenings, weekends, and UK bank holidays
